Such processing may be for the purpose of creating a personal account, booking an appointment and browsing the Practice's Practitioner(s) Site. The data controller is referred to in this Policy as "we" or "Health Care Professional". Dr. Adriana Agachi, a private dental surgeon, practicing at 5 rue Lalo, 75116 Paris, France, referred to in this Policy as " Manager "It acts on the specific instructions of each Health Professional.


3. What personal data do we process?

We may collect and process the following personal data when you browse the Site, on Doctolib, or when you fill in our online questionnaires:

  • identification data: name, first name, date of birth, postal address, e-mail address, telephone number;
  • medical history: history, health professionals;
  • connection and location data: IP address, connection time stamp, geolocation;
  • Data related to your appointment with the Health Care Professional: reason, time and history of appointments
  • the same data about your loved ones for whom you wish to make an appointment.

Only personal data that is strictly necessary for the purposes described below is collected.


4. What are the purposes of the processing?

The personal data collected are processed for the following purposes:

  • your connection with health professionals;
  • the management of your appointments with the Health Professionals (example: to obtain notifications, to modify them, to cancel the appointments) and your care course;
  • the management of your loved ones' appointments with health professionals and their care path;
  • your efficient and optimal care by the Health Professionals; but also, as a subsidiary
  • Improved knowledge of your profile, behavioral analysis ;
  • personalizing our services according to profiles and improving your experience;
  • statistics, analysis and audience measurement;
  • satisfaction surveys on our services;
  • to improve and optimize the quality of our services and our site;
  • management of rights and requests to exercise the rights of individuals.

5. On what legal grounds do we process your personal data?

Personal data are collected and processed on the following legal bases:

  • on the basis of your consent when it is required by the regulations in force, in particular when we collect your health data;

6. Who are the recipients of your personal data?

Your personal data is strictly for :

  • to the Health Professionals concerned;
  • to the approved health data host, exclusively for the purposes mentioned above.

You are informed that the health data are in no case transmitted to a third party. In accordance with the provisions of Article L. 1111-8 of the Public Health Code, the health data of Patients are hosted by a service provider approved for this purpose by the Ministry of Health. 


7. How long do you keep your personal data?

Your personal data will only be kept for the time strictly necessary to achieve the purposes for which they are collected and processed, in accordance with legal requirements.


8. What are your rights regarding your personal data?

Your rights regarding your personal data are as follows:

  • right of access ;
  • right of inquiry;
  • right of rectification ;
  • right to erasure - this right may be exercised to the extent that it does not interfere with the performance of the contract or with our legal and regulatory obligations;
  • right to limit one or more processing operations of your personal data;

You can also speak directly to the health care professional, or write to him/her at the address of the office, specified below. In the event of a request for deletion of your personal data, the healthcare professional reserves the right to retain certain categories of data that may be necessary, in the event of a subsequent claim or dispute, for the legally permitted retention period. However, this data will be deactivated and will no longer be accessible online.

9. What about cookies?
